Teacher's Allergy Notice

Consider giving your child's teacher an allergy notice that contains specific information about your child's allergies as well as his or her photograph.  It will make it easier, and safer, when a substitute teacher is supervising your child's class--she (or he) can see the child's face along with the allergies and likely symptoms of a reaction.  Here's a sample form:
